Hi, I’m Brett Roberts, APRN — an Advanced Practice Registered Nurse with five years of experience working closely with people living with diabetes.

I started this virtual service after seeing too many patients overwhelmed by blood sugar numbers, restrictive diets, and rushed clinic visits. Diabetes management shouldn’t feel like a constant battle and it definitely shouldn’t be something you have to figure out alone.

Growing up in a rural New Hampshire community with limited healthcare resources, I saw firsthand how a lack of access to specialized care can impact families — including my own. That experience shaped my commitment to making diabetes support more accessible, especially in underserved communities where care options are often scarce.

I attended Keene State College in Keene, NH where I earned my Bachelor’s Degree in Nursing, and later returned to MCPHS University in Boston, MA to complete my Master’s Degree as a Family Nurse Practitioner.

My clinical interests are rooted in diabetes, metabolism, and weight management. I’m passionate about helping people make practical, sustainable lifestyle changes that improve their health and quality of life.

My approach is practical, evidence-based, and tailored to your personal goals. Whether you’re newly diagnosed or navigating diabetes long-term, my mission is to help you feel empowered, supported, and heard — every step of the way.

Derek serves as our dedicated Nurse Care Manager at NextGen Diabetes Care, where he plays a vital role in supporting individuals living with diabetes. Through our innovative Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, Derek helps patients stay closely connected with their care teams, improving communication and continuity of care.

With a compassionate approach and a strong clinical background, Derek guides patients in navigating the complexities of the healthcare system, ensuring they have the resources and support needed to manage their condition effectively. By fostering consistent engagement and proactive monitoring, Derek helps reduce the risk of complications, hospitalizations, and illness; empowering individuals to take control of their health and improve their quality of life.
